Terms and Conditions

Skin Deep RX

Effective date: October 22, 2026

 

These Terms and Conditions (“Terms”) govern your use of skindeeprxutah.com (the “Site”) and your scheduling of services with Skin Deep RX (“Skin Deep RX,” “we,” “us,” or “our”), located at 1102 W. 60 S., Suite 4, Tooele, Utah 84074. By using the Site or booking an appointment, you agree to these Terms. If you do not agree, do not use the Site.

1. No Medical Advice; No Provider-Patient Relationship

All content on the Site, including service descriptions, blog posts, webinars, and linked social media content, is for general educational purposes only and is not medical advice, diagnosis, or treatment. Using the Site, submitting a contact form, or messaging our chat assistant does not create a provider-patient relationship. That relationship begins only when you complete an intake and are evaluated by one of our licensed providers. Never disregard professional medical advice or delay seeking it because of something you read on the Site.

2. Not for Emergencies

The Site, contact form, chat assistant, and email are not monitored for urgent matters. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room.

3. Eligibility

You must be at least 18 years old to use the Site or book services. We may require proof of age and identity before treatment. Certain services have additional medical eligibility criteria determined by your provider.

4. Appointments and Scheduling

Appointments are scheduled through a third-party scheduling and electronic health record platform, and your use of that platform is also subject to its own terms and privacy policy. Booking an appointment does not guarantee that a particular treatment will be provided. All treatment decisions are made by your provider after evaluation.

Appointment deposit: A $50 deposit is required to book an appointment. The deposit is applied in full toward the cost of your visit.

Cancellations and rescheduling: If you cancel at least 24 hours before your scheduled appointment time, your deposit will be refunded in full. If you reschedule at least 24 hours in advance, your deposit carries over and is applied to your new appointment.

No-shows and late cancellations: If you do not show up for your appointment, or you cancel or reschedule less than 24 hours before it, the $50 deposit is forfeited and will not be refunded or applied to a future visit.

5. Pricing and Payment

Skin Deep RX is a cash-pay practice. We do not bill insurance, including Medicare or Medicaid, for our services. Payment is due when services are rendered unless otherwise agreed in writing. Prices shown on the Site are for general reference and may change without notice. The price that applies is the one quoted to you at the time of service or enrollment.

Where a program is described as a flat monthly rate, that rate covers the items stated in your program agreement and does not increase with dose changes during the enrolled period. Lab work, supplies, and services outside your program are billed separately unless stated otherwise. If you choose to seek reimbursement from an insurer or a health savings or flexible spending account, you are responsible for doing so, and we do not guarantee reimbursement.

6. Refunds

Appointment deposits are governed by Section 4. Services already performed are non-refundable. For patient safety and under pharmacy regulations, medications, injectables, and supplies are non-returnable and non-refundable once dispensed. Program fees for future periods may be cancelled according to the terms of your program agreement. Questions about a charge should be directed to us within 30 days of the charge.

7. Prescriptions and Treatment

Whether to prescribe any medication, and at what dose, is solely at the professional discretion of your provider, based on your history, labs, and response to treatment. Requesting or paying for a consultation does not entitle you to any particular prescription. Your provider may modify, pause, or discontinue treatment when medically appropriate.

Compounded medications: Some medications we prescribe may be prepared by licensed compounding pharmacies. Compounded medications are not FDA-approved, and the FDA does not review their safety, effectiveness, or quality before they are dispensed. Your provider will discuss whether a compounded or commercially manufactured medication is appropriate for you.

Before receiving treatment, you will be asked to sign treatment-specific informed consent forms. Those consent forms, together with any program agreement you sign, govern your clinical care and control over these Terms if there is a conflict.

8. Results Not Guaranteed

Individual results vary. Weight loss, hormone optimization, and aesthetic outcomes depend on many factors, including your physiology, adherence to your treatment plan, nutrition, activity, and medical history. Any results, timelines, or testimonials described on the Site are not a promise or guarantee that you will achieve the same results.

Testimonials: Reviews on the Site reflect the individual experiences of real patients and are not typical or guaranteed outcomes. We do not pay for reviews.

9. Privacy and Health Information

Our Privacy Policy explains how we handle information collected through the Site. Our Notice of Privacy Practices explains how we use and protect your protected health information as a HIPAA-covered provider.

Do not send health information through the Site’s contact form, chat assistant, or standard email. These channels are not designed for confidential medical communication. Please use our patient portal or call our office.

10. Communications

By providing your phone number or email, you agree that we may contact you about your appointments, treatment, and account, including by text message. Appointment reminders and treatment-related messages are part of your care. Promotional messages, such as event and webinar announcements, are sent only with your separate consent. You may opt out of promotional texts at any time by replying STOP, and out of promotional emails by using the unsubscribe link. Message and data rates may apply.

11. Events and Webinars

Our seminars, webinars, and community events are educational only. They do not establish a provider-patient relationship and do not provide individualized treatment, dosing, or usage guidance. Any discussion of medications or peptides is general information. Treatment decisions require a consultation with a licensed provider.

12. Intellectual Property

All Site content, including text, graphics, logos, photographs, videos, and design, is owned by or licensed to Skin Deep RX and is protected by U.S. copyright and trademark law. You may view and print content for personal, non-commercial use. You may not copy, reproduce, republish, or distribute it without our written permission.

13. Acceptable Use

You agree not to:

  • use the Site for any unlawful purpose;

  • submit false information or impersonate another person;

  • interfere with the Site’s operation or security;

  • attempt to access systems or data you are not authorized to access;

  • scrape or harvest Site content or user information;

  • post content that is defamatory, harassing, or infringing.

14. Third-Party Links and Services

The Site links to third-party services, including our scheduling platform, social media, maps, and pharmacies. We do not control and are not responsible for their content, policies, or practices. Your use of them is at your own risk and subject to their terms.

15. Disclaimer of Warranties

THE SITE AND ITS CONTENT ARE PROVIDED “AS IS” AND “AS AVAILABLE,” WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, ACCURACY, AND NON-INFRINGEMENT. We do not warrant that the Site will be uninterrupted, error-free, or free of viruses or other harmful components.

16. Limitation of Liability

TO THE FULLEST EXTENT PERMITTED BY LAW, SKIN DEEP RX AND ITS OWNERS, PROVIDERS, EMPLOYEES, AND AGENTS WILL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING FROM YOUR USE OF, OR INABILITY TO USE, THE SITE. OUR TOTAL LIABILITY FOR ANY CLAIM ARISING FROM YOUR USE OF THE SITE WILL NOT EXCEED ONE HUNDRED DOLLARS ($100).

This section applies only to your use of the Site. It does not limit any liability for professional medical services that cannot be limited under Utah law. Claims relating to medical care are governed by the Utah Health Care Malpractice Act.

17. Indemnification

You agree to indemnify and hold harmless Skin Deep RX and its owners, providers, employees, and agents from any claims, losses, or expenses, including reasonable attorneys’ fees, arising from your violation of these Terms or your misuse of the Site.

18. Governing Law and Venue

These Terms are governed by the laws of the State of Utah, without regard to its conflict-of-law rules. Any dispute arising from these Terms or your use of the Site will be brought exclusively in the state courts located in Tooele County, Utah, or the federal courts for the District of Utah, and you consent to their jurisdiction.

19. Changes to These Terms

We may update these Terms at any time by posting the revised version on this page with a new effective date. Continued use of the Site after changes are posted means you accept the revised Terms.

20. General

If any provision of these Terms is found unenforceable, the remaining provisions remain in full effect. Our failure to enforce any provision is not a waiver of it. These Terms, together with our Privacy Policy and any consent forms or program agreements you sign, are the entire agreement between you and Skin Deep RX regarding the Site.
