-
information about treatment options or services we offer that may interest you.
Business associates. We work with outside companies that perform services for us, such as our electronic health record and scheduling platform, laboratories, and IT providers. They are required by contract to protect your information.

Your Rights
Get a copy of your health information. You may ask to see or get a paper or electronic copy of your medical record. We will provide it within 30 days. We may charge a reasonable, cost-based fee.
Ask us to correct your record. If you believe information is incorrect or incomplete, you may ask us in writing to amend it. We may deny the request, and if we do we will explain why in writing within 60 days.
Request confidential communications. You may ask us to contact you in a specific way, such as by cell phone only or at a different address. We will agree to all reasonable requests.
Ask us to limit what we use or share. You may ask us not to use or share certain information. We are not required to agree. If you pay for a service or item in full out of pocket, you may ask us not to share that information with your health insurer, and we will agree unless the law requires us to share it.
Get a list of those we’ve shared information with. You may ask for an accounting of certain disclosures we made in the six years before your request.
