Skin Deep RX (“we,” “us,” or “our”) operates skindeeprxutah.com. This policy explains what information we collect through our website, how we use it, and what choices you have. Please do not send urgent medical concerns through our website — call the office at (435) 274-8897 instead.

Appointment and payment information. Scheduling and payment are handled through third-party platforms. When you book or pay, your information is collected and stored by those providers under their own privacy policies, not ours.

Information collected automatically. Like most websites, ours records basic technical data when you visit: IP address, browser type, device type, pages viewed, and how you arrived at the site. This is collected through cookies and similar technologies.

With service providers who help us operate — our scheduling platform, payment processor, email service, and website host. They may only use the information to perform services for us.
With pharmacies and labs involved in your care, as needed to fill prescriptions or process orders you’ve authorized.
When the law requires it in response to a subpoena, court order, or lawful request from a regulator or law enforcement agency.
To protect people if we believe disclosure is necessary to prevent serious harm to you or someone else.

How Long We Keep Information

We retain website inquiries and contact information for as long as needed to serve you and to meet our legal and recordkeeping obligations. Medical records are retained according to Utah law and professional standards, which is separate from and longer than the retention period for general website data.

Your Choices and Rights

Marketing messages. You can unsubscribe from emails using the link at the bottom of any message, or reply STOP to text messages. You’ll still receive appointment-related communications.

Access and correction. You may ask us what personal information we hold about you, request corrections, or request deletion. Some information we’re required to keep — medical records in particular — and we’ll tell you if that applies.

Utah residents. Under the Utah Consumer Privacy Act, you have the right to confirm whether we process your personal data, obtain a copy of it, delete data you provided to us, and opt out of the sale of personal data or targeted advertising. To exercise any of these, contact us using the information below. We’ll respond within the timeframe the law requires.

Do Not Track. Our site does not currently respond to browser Do Not Track signals.

Children’s Privacy

Our website and services are not directed to children under 13, and we do not knowingly collect information from them. If you believe a child has provided us information, contact us and we’ll delete it.

Changes to This Policy
