Privacy Policy

Skin Deep RX Effective date: 06/28/2026

Skin Deep RX (“we,” “us,” or “our”) operates skindeeprxutah.com. This policy explains what information we collect through our website, how we use it, and what choices you have. Please read it alongside our Notice of Privacy Practices, which covers the protected health information we handle as part of your care.

Information We Collect

Information you give us directly. When you book an appointment, fill out a contact form, sign up for our newsletter, or create a member account, you may provide your name, email address, phone number, and any details you choose to include in a message to us.

Health information. If you tell us about a health concern, a medication, a treatment you’re interested in, or anything similar through a web form or message, we treat that as health information and protect it accordingly. Please do not send urgent medical concerns through our website — call the office at (435) 274-8897 instead.

Appointment and payment information. Scheduling and payment are handled through third-party platforms. When you book or pay, your information is collected and stored by those providers under their own privacy policies, not ours.

Information collected automatically. Like most websites, ours records basic technical data when you visit: IP address, browser type, device type, pages viewed, and how you arrived at the site. This is collected through cookies and similar technologies.

How We Use Your Information

We use the information we collect to:

  • Schedule, confirm, and follow up on appointments

  • Respond to questions and requests you send us

  • Provide and improve our services

  • Send appointment reminders and, if you’ve opted in, promotional messages

  • Understand how visitors use our site so we can make it better

  • Meet our legal, regulatory, and professional obligations

What We Do Not Do

We do not sell your personal information. We never have and we don’t intend to.

We do not share your health information for advertising. We do not use health information you provide through this site to target ads to you, and we do not pass it to advertising networks or social media platforms.

We do not share your information with third parties for their own marketing purposes.

When We Share Information

We share information only in these situations:

  • With service providers who help us operate — our scheduling platform, payment processor, email service, and website host. They may only use the information to perform services for us.

  • With pharmacies and labs involved in your care, as needed to fill prescriptions or process orders you’ve authorized.

  • When the law requires it in response to a subpoena, court order, or lawful request from a regulator or law enforcement agency.

  • To protect people if we believe disclosure is necessary to prevent serious harm to you or someone else.

  • In a business transfer if our practice is sold or merged, your information may transfer as part of that transaction, subject to the same protections described here.

Cookies and Tracking

Our site uses cookies to keep it functioning, remember your preferences, and measure traffic. Most browsers let you refuse cookies or alert you when one is set. Blocking cookies may limit some features of the site.

We do not place advertising or social media tracking pixels on pages where you submit health information.

Third-Party Services

Our website links to and integrates with outside services, including our online scheduling system and our social media pages. Once you leave our site or submit information through one of those platforms, this policy no longer applies. We encourage you to review the privacy policies of any service you use.

Data Security

We use reasonable administrative, technical, and physical safeguards to protect the information we hold. No system is completely secure, though, and we can’t guarantee that information transmitted over the internet is fully protected. Sending sensitive details by email or web form always carries some risk.

How Long We Keep Information

We retain website inquiries and contact information for as long as needed to serve you and to meet our legal and recordkeeping obligations. Medical records are retained according to Utah law and professional standards, which is separate from and longer than the retention period for general website data.

Your Choices and Rights

Marketing messages. You can unsubscribe from emails using the link at the bottom of any message, or reply STOP to text messages. You’ll still receive appointment-related communications.

Access and correction. You may ask us what personal information we hold about you, request corrections, or request deletion. Some information we’re required to keep — medical records in particular — and we’ll tell you if that applies.

Utah residents. Under the Utah Consumer Privacy Act, you have the right to confirm whether we process your personal data, obtain a copy of it, delete data you provided to us, and opt out of the sale of personal data or targeted advertising. To exercise any of these, contact us using the information below. We’ll respond within the timeframe the law requires.

Do Not Track. Our site does not currently respond to browser Do Not Track signals.

Children’s Privacy

Our website and services are not directed to children under 13, and we do not knowingly collect information from them. If you believe a child has provided us information, contact us and we’ll delete it.

Changes to This Policy

We may update this policy from time to time. When we do, we’ll revise the effective date at the top. Material changes will be noted on this page.
